Fix up pcmcia

This really moves us into line with the current pcmcia code (and corrects
a few errors).

NOTE: We now have to add a specific set of include lines to
/etc/pcmcia/config.opts

These should be

include memory 0xf0000000-0xffffffff
include io 0x0-0x7fff
