Most changes are for preparation to get ioctl(GCDESCRIBE) working
(needed to get XFree86 running):

- extraversion: pa3
- added union struct for regions (region_t)
- #define REGION_OFFSET_TO_PHYS() for easy access to region addr
- replaced u32 with region_t where appropiate
- make stifb screensaver work with blank_palette hack
- added new graphics id entries
