Yet another intermediate version:
o more debug info around CPU rendezvous
o disabled flush_tlb_all() and flush_cache_all() slave path for now
o "adjust" cpunum vs cpuid usage. I need clarification on when each
should be used (logical vs "physical" CPU naming).
